By completing this form you will be placed on a list of potential attendees for our Level Up Boot Camp at The Hit Davie -Pembroke Pines. Our front desk will be contacting you to confirm your participation and to process your $100 non-refundable deposit which is due at registration.

The total cost of the two-week camp is  $590. All students must enroll in both weeks as the curriculum will be cumulative.)
